_*Job Description*_ *TITLE:* QIDP/ Case Manager Child Welfare Worker Quality Assurance Team Member *SECONDARY TITLES UNDER THIS DESCRIPTION:* N/A *REPORTS TO:* Program Director *PAY CLASSIFICATION:* Non-Exempt (Hourly) *Effective Date:* 2/13/2023 *REQUIREMENTS:* *EDUCATION:* Bachelor’s Degree in Human Services *LICENSURE/ CERTIFICATION:* QIDP Certification Current in CPR/ First Aide *EXPERIENCE:* One year of full-time experience with individuals with developmental disabilities *PHYSICAL REQUIREMENTS:* Able to assist with transfers, self-care needs, self-preservation, and behavioral interventions.
This may include: bending, twisting, stooping, squatting, pushing, pulling, standing, sitting, crawling, reaching, walking, driving, traveling, kneeling, and crouching.
*OTHER (AS PERTAINS TO POSITION):* Valid Driver’s License and MVR approval At least 25 years of age for Child Care Facility *DUTIES AND RESPONSIBILITIES:* - All duties as outlined in the “Basic Qualifications” Section of the Five Star employee manual (section 1.
04) or the section titled “General Job Description for all staff” in the Temp.
Staff Manual - *Client Needs:* o Assessment: § Coordinate assessment of client needs and wants including functional limitation, strengths and needs, level of functioning, presenting problem, disability and diagnosis § Communicate/ explain assessments so they can be understood § Assess the needs of persons served and access the resources to meet needs o Program Plans: § Develop, monitor, and document implementation of the Implementation Strategy Plan (based off of the Personal Plan and Discovery Tool completed by the ISC); evaluate and revise as necessary to meet client needs.
§ Develop and implement behavioral programming techniques as needed · Work with behavior analyst to develop programming as needed § Monitor and document the progress of clients § Provide training for staff specific to client needs/ plans o Monitor the health and safety of each client § Assist client in making a personal selection of programs, doctors, counselors, and residences as appropriate § Assist client in obtaining and maintaining appropriate health care § Assist client in obtaining and maintaining appropriate nutritional status o Client Finances: § Assist clients in obtaining and maintaining financial resources § Report financial activity of clients to their family and/ or guardians on a quarterly basis § Assist clients with banking skills § Follow client financial procedures as per Financial/ Accounting Policies and Procedures § Process all appropriate requests for client funds o Client General: § Coordinate and document admission, discharge, and transfer of clients § Orient clients to new programs § Maintain content and confidentiality of client records § Arrange for maintaining support in periods of crisis or transition § Assist client in developing natural supports in the community § Assist client in advocating for their rights, as necessary § Provide case consultation and supervision § Provides transportation of residents/ clients as needed to community outings, medical appointments, etc.
§ Communicate with and provide pertinent info to ISC agency § Assist client with maintaining all benefits (Medicaid, SNAP, Social Security, etc.
) - *Programs:* o Maintain and enforce all rules pertaining to the programs o Maintain and implement ongoing compliance with federal and state regulations/ standards including but not limited to Medicaid Waiver, COA, etc.
o Have knowledge of the range of service interventions o Meet Quality Assurance and other evaluation requirements § Collect and apply data to outcomes - *General Duties:* o Attends all training as assigned and completes yearly CEU requirements for QIDPs o Attends meetings as assigned, providing input and ideas o Responds to emergency situations as needed o Develop and maintain positive relationships with clients, families, guardians, and other agencies o Implement all emergency procedures in the event of a fire, earthquake, other natural disasters, medical emergencies, etc.
- *Staff Supervision:* o Provide supervision of staff assigned to you to manage by completing disciplinary action, evaluations, check-ins, etc.
o Assign duties to staff as needed o Coordinate staff time off as needed o Help make recommendations about staff to upper management - On-call duties as assigned - Perform other duties/ responsibilities as assigned Job Type: Full-time Pay: From $24.
